*Request for Proposals (RFP) For Quality Improvement*

*DETAILS: *The Minnesota Department of Administration, Governor?s Council on Developmental Disabilities (GCDD) is requesting proposals for the purpose of completing quality improvement on behalf of the GCDD.

The selected contractor will assist with the development of a quality improvement strategy that will be applied to all GCDD products and services including Five Year State Plan logic models for each goal, the ACL annual Program Performance Report, the GCDD?s annual Work Plan, Business Results, Annual Reports, and monthly reports. The initial contract will be in the amount of $20,000.00 and the contract may renew up to four additional years.
