HHSC welcomes STAR Kids managed care organizations and providers to participate in the Comprehensive Health Homes for Integrated Care Kids Pilot program. The pilot will focus on members of the STAR Kids program who have multiple chronic conditions and are 20 years old or younger.
The goals of the pilot are to:
- Provide enhanced care coordination through a health home specifically designed for children with medically complex conditions.
- Improve access to care, health outcomes and member satisfaction among children with medically complex conditions and their families.
- Reduce administrative burden for MCOs and providers.
To participate, MCOs and providers must partner in developing project plans to create or enhance a health home model for children with medically complex conditions. Project plans must be submitted by MCOs to HHSC CHIC Kids for review and approval by Aug. 12.
All participation is voluntary for MCOs, providers and members. The pilot will run from Dec. 1 until Dec. 31, 2023.
