Social Isolation and Loneliness Livable Communities Request for Applications

The Department of Health Services (DHS) is pleased to announce this grant opportunity for agencies interested in improving social connections within their community to address or avoid the health consequences of social isolation and loneliness for older adults and people with disabilities. This opportunity is intended to provide local communities with the resources to form or enhance a local coalition and develop innovative and relevant solutions that address the unique needs of local populations and communities. Awardees will be expected to work collaboratively with a relevant and diverse base of local partners in understanding current resources for supporting older adults and people with disabilities, developing prevention strategies, identifying gaps and needs, and creating local solutions.

Eligible applicants

Public and private organizations that have the demonstrated capacity to engage community partners and that have experience or knowledge pertaining to the unique social needs of older adults and people with disabilities are encouraged to apply.

Grant deliverables

Selected agencies will:

  • Form or enhance a local coalition focused on social connection.
  • Involve a range of partners that reflect the diversity of the community served.
  • Identify the unmet needs and current capacity of the community service area to respond to those needs.
  • Gather data elements, report on outcomes, and share experiences and lessons learned with DHS, other coalitions, and interested groups.
  • Optionally, identify and implement a community project that will substantively reduce social Isolation and loneliness.
  • Develop a plan for opening or enhancing a certified adult day care center targeting people living with dementia.
  • Plan for sustainability of all efforts after the grant ends.

All awardees' agencies will receive reimbursement for administrative and startup costs.

Grant funding

The Social Isolation and Loneliness and Livable Communities grants are possible thanks to federal funding from the American Rescue Plan Act. All funds must be spent by March 31, 2025.
