Amy Finkelstein of the Massachusetts Institute of Technology and her team of coauthors explore the results of the Camden Coalition’s Camden Core Model in the upcoming January issue of Health Affairs.
The Camden Core Model is a care coordination model which targets high-use, high-needs patients in Camden, New Jersey, and provides high-touch, appropriate care, with the ultimate goal of reducing hospitalizations, improving patient well-being, and reducing cost.
Randomized evaluations of the Camden Coalition’s program did not find evidence of reduced hospital readmissions; the authors explore explanations for this null result.
They conclude that “care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this complex population of high-needs patients.”
